We are here to answer your questions and go over treatment options and fee for services. ​

Or have your provider fax over your referral for specialized pediatric physical therapy services.

Intake paperwork is provided via a HIPPA compliant portal for easy access to provided detailed information on your child's medical information, health history and goals.
​
Evaluations include assessment, recommendations, and plan of care to meet your child's goals. Evaluations are required for new clients prior to starting therapy services.
Children are treated at IN FOCUS for optimal development of the musculoskeletal and neuromuscular systems for lifetime growth and wellness.
